Class Booking

Class Bookings are limited to only 10 people per class. Depending on the class package you choose please kindly choose the classes you WILL be attending so others can also enjoy our classes.
Please complete the COVID 19 Waiver and Registration Waiver before attending to class.

    COVID 19 Guidance

    By signing this agreement, i acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that I and/or my child may be exposed to or infected by COVID-19 by attending the studio and that such exposure or infection may result in personal injury, illness, permanent disability, and death.

    I understand that the risk of becoming exposed to or infected by COVID-19 at the studio may result from the actions omissions or negligence of myself and others, included but not limited to, staff and program participants and their families.

    Azabache is doing everything we can to protect you the public as well as our workers and volunteers. To this extent, we will be following the Center of Disease Control (CDC) and local health departments guidelines with regard to social distancing practices in order to reduce the spread of Novel Coronavirus, or COVID-19. This will require our staff and volunteers to maintain six(6) feet of distance between ourselves and you, the client as much possible. We will also be requiring this same procedure for client-to-client contact in order to limit the exposure to all individuals.

      LIABILITY WAIVER

      I understand and agree that in participating in any dance class, workshop, rehearsal or performance, there is a possibility of physical injury or death. I voluntarily agree, therefore, to assume all risks and responsibility for any such injury or accident, which might occur to me or my child during any of Azabache Dance Studio classes, rehearsals, performances, or activities. I also exempt, release, and indemnify Azabache Dance Studio, its owners, agents, volunteers, assistants, employees, guest artists, faculty members, and/or students from any and all liability claims, demands, or causes of action whatsoever from any damage, loss, injury, or death to me, my children, or property which may arise out of or in connection with participation in any classes or activities conducted by Azabache Dance Studio. I further hereby voluntarily agree to waive my rights and that of my heirs and assigns to hold Azabache Dance Studio, its owners, agents, volunteers, assistants, employees, guest artists, faculty members, and/or students liable for such damage, loss, injury, or death. I understand that I should be aware of my physical limitations and agree not to exceed them. If I am signing this waiver for my children, I certify that I am the parent or legal guardian and have the right to waive these rights.

      CANCELLATION POLICY

      Must email us 2 weeks prior to your billing date at info@azabachedancestudio.com
      No cancellations will be taken over the phone or in person.
      Cancellation fee: equal to one-month membership after 3 months of being in membership.

      Permission is granted Azabache Dance Studio to use photographs and/or videos of students for publicity purposes.
